How do I obtain my certificate?

  • Mississippi State University Extension Service will mail your certificate to the address you provided when you took the class.  It can be mailed to your home, your business or employer, or an address of your choosing.  Please make sure this address is correct to avoid your certificate possibly getting lost.  If a replacement certificate is needed, the cost is $20 for each certificate requested.

  • If you have lost your certificate and in need of a duplicate copy, it is easiest to go through the ServSafe website to request a duplicate copy. You may request one by sending a completed Certificate Request Form to National Restaurant Association Solutions.

  • Download, print out, and complete the Certificate and Score Release Request Form. Downloading this PDF file requires installation of Adobe® Acrobat®Reader®. Print legibly when completing the form.

How to download: Save the file to your computer's hard drive and open from there rather than opening it via your web browser.
In Internet Explorer, right click on the PDF link; choose "Save Target As..."; save the file to the location you want and then open the file once the download finishes.
In Netscape Navigator, right click on the PDF link; choose "Save Link As... "; save the file to the location you want and then open the file once the download finishes.

How do I request a correction or change of information on my certificate?

To request a correction or to change information on your certificate, submit a Certificate Request Form to ServSafe. The cost is $20 for each certificate requested.

What is the Revocation Policy and procedure?

National Restaurant Association Solutions reserves the right to revoke an examinee's certificate based on any security violation that severely compromises the validity, reliability, security or integrity of the National Restaurant Association Solutions Certification examination programs.

Any candidate or examinee whose certificate has been revoked, may appeal a denial or revocation by submitting a written request to the Certification Department within thirty (30) calendar days of notification of the denial or revocation.

Upon receipt of the appeal, the Exam Administration Department forwards the request with all prior documentation (if any) to the Director of Exam Administration or Director of Exam Development (depending on the nature of the concern) for review. The Director can escalate the concern to the Certification Governing Board, if warranted, or will review and make a final decision on the appeal within 30 days of receipt of the written request. This final decision will be communicated in writing, via traceable mail, to the individual who submitted the appeal within ten (10) days of rendering the decision.

The decision of the Exam Director or Certification Governing Board concerning all appeals is final.
